what is the sigma 1 receptor purpose as it appears that luvox is the only ssri/snri to hit that receptor .....what is its clinical significance?

Fluvoxamine activates it most potently followed by citalopram and fluoxetine. It is involved in calcium homeostasis, neurogenesis and neuroprotection.
Sigma agonists, on their own, exhibit antidepressant and anti-anxiety effects. They also have the ability to improve cognition. Fluvoxamine, for instance, can reverse the cognitive impairment from a variety of drugs (like anticholinergics) while other antidepressants don't seem to do this.
The natural hormone DHEA is a sigma agonist, whereas progesterone is a sigma antagonist.
