Prozac stays in your bloodstream much longer than other SSRI's, so I don't quite see what good it would do to discontinue it for surgery. It usually takes a month or so to disappear.
As to the medication not working a second time around - it varies. I went off Prozac (and all other AD's) for six months and then resumed use with the same benefits. I've talked to others who had the opposite result - it didn't work or at least not as well.
