This is probably the most important MAOI-related thread I've seen in the 2+ years I've been following Psychobabble. Most of the time, people talk about the diet restrictions, and sometimes contraindicated medications (OTC cold remedies). But I don't think we've had anything like this and it's something that's just as important if not more, than the hypertensive crisis concerns. I think that most of us MAOI patients, myself included, like to think, "oh this couldn't happen to me," i.e. we won't have to face the complications of anaesthesia from emergency surgery.
As a long-time believer in MAOIs, I wouldn't want this particular drawback of Parnate/Nardil to deter anyone from trying these meds, but we all have to think that emergencies can happen, get armed with all the facts,and be sure to impart all we know to the surgeon and anaesthesia personnel. I'd never be complacent about it or assume that they know.
Living here in Thailand, where MAOIs are not even available and some doctors are virtually ignorant about them, I've had to carry around detailed data on these drugs in the Thai language for emergency use.
