It seemed missing....

IMHO, if one's gotten to the point where drugs are necessary to cope with life, one might as well pull out all the stops. I wish I'd been given Nardil right off the bat. Who cares about whether you can eat soy sauce if the only thing you think about eating is the business end of a handgun?Soy sauce, soybeans, tofu... but then I'm a vegetarian... and it goes on... RIMAs might not be a last resort as MAOIs should be, but you just don't jump to MAOIs. I'm glad you are fine with Nardil but interactions with it, and overdosing on it especially -- will lead to comatose states. Is that living ?

IMHO, there are many cases where they still should be.And there was a time when people were stuck with insulin in the funny farm and held down with withstraints, and Kellogg believed in the bowels, along the way Corn Flakes(tm) came by.
Star*D is a fascinating insight into protocols for treatment resistant depression, and you might look into it because yes, it does include MAOIs, but that's not the beginning of the maze. Years of research produced something that looks like a promising idea.
