It may be it's just simply not going to work the second time around. The first time I was on Parnate it worked for depression at 20 mg..but hypotension was so bad I had to discontinue (long story)..the second time around I went to 50 mg..not even a partial response for depression and blood pressure was all over the place...mostly in the high range.
For me, this happens all the time. Revisiting meds is frustrating. They may work again but with a whole different set of side effects, or don't work at all.
What does your pdoc think? My experience with MAOI's is the higher the dose, the worsening of side effects, although many have different experiences.
I guess it comes down to the type of side effects you're prepared to tolerate to give the medication a trial..I hope you discuss it with your doctor..we can give our experiences and support on babble, but we're not walking in your shoes..severe agitation and anxiety is awful to deal with. I hope you get some relief soon.
