You can be assured that you won't be telling your doctor anything he hasn't heard or, more importantly, needs to hear from you :-)
I can make a couple of suggestions:
Write your notes out beforehand of what you plan to tell him, i.e., what you've been prescribed versus what you need to take to be effective.
Have a trusted member of your family or a close friend who knows what's going on accompany you.
I think that the role of shame in the doctor-patient relationship, once it is dealt with and is out in the open, can lead to a much better therapeutic treatment plan for you. If, on the other hand, you fear recriminations and maybe getting tossed from the practice (which I rather doubt will happen), then I suggest that you're not seeing the right doctor.
