no. not going nuts. sounds like you have a lot of stressors right now.

i would tell pdoc about rashes, but it might be more of a GP thing rather than pdoc. especially if you have not added anything new or changed anything around time it began.
without seeing it i would guess its a contact rash of somesort.
with one watch i used to get slight rash under, turned out i was allergic to something in the material. maybe your forearms are touching a new cleaner (say when you are typing on the computer) or new soap etc.
that would explain also why they appear and reappear.

either that or they are stress like hives.

normally if you gain a rash from medications they happen either on the torso or face/neck area.

either way. wouldnt hurt to mention to doc. (either pdoc or gp).

your triggers, your ex, you dog, fear of cancer, sorrow for bird, these are all very real and concerning but also anxiety driven stressors.
work on anxiety. maybe non medication or combination when these things happen to you. journal them, or listen to relaxing music or meditation sounds.
idk. might not help but sure worth a try.
