Many of the cardiovascular issues are likely caused by a large surge in norepinephrine via reuptake inhibition and/or release. Some of it could be related to sodium channel blockade at high doses. Venlafaxine IR for example has far more cardiovascular side effects than the XR version for (perhaps) a similar reason. A massive and sudden inhibition of the norepinephrine transporter can likely put your blood pressure through the roof. Again, I don't know whether this would be an issue with an XR version.
