I'm sure you've already thought of this, but could the GP you saw give you some Provigil? It's a pretty loosely-controlled med, and you're accustomed to taking it, so it's not exactly a high-risk situation for you, him or her.
If you were to call the GP's office and explain that you're in a bad situation, having been taken off a med you need, due to a mixup (and mention that you haven't received the list of pdocs they promised), just maybe you could get a prescription. Or, if you're lucky, they might even have samples -- there are lots around.
It's a long shot, but worth a try. I once (through sheer persistence/insistence) persuaded a stand-in GP who was clearly benzophobic to prescribe Xanax when I really needed it and my regular doctor was on vacation.
