Generally, with metabolized drugs such as desvenlafaxine (Pristiq)--> Effexor (venlafaxine) or paliperidone (Invega) --> risperidone (Risperdal); also, you have prodrugs such as Vyvanse, oxcarbamazepine (Trileptal) [that removes the epoxide away from the carbazepine for better side effects] or various pain management derivatives; their effects are going to mimic their master medicament and be clinically biased as such.

However, these medicines can be sneaky and alternative effects, some significant and some mild, have been observed from all that differentiate from the master medicament.

Desvenlafaxine is definitely one that once patients start-- they do not handle Effexor well at all. Anecdotal note: both my dad and I took Pristiq (both diagnosed with MDD, he was indicated for Pristiq, responded, and I took it due to indication of a genetic response) and we both decided to taper using Effexor XR-- well, we definitely got the F off of it.

With the changeover from Pristiq to Effexor XR during a titration off of it, I developed anorgasmia from it that I did not have with Pristiq as well as a "blunted effect", if that makes sense.

** disregard MY THEORY! When Pristiq users begin to start taking the Effexor they can only get their CYP2D6 enzymes to get about 3/4 of the venlafaxine converted into Pristiq. The other 1/4 venlafaxine BS, the alternative metabolites, might be causing different effects such as stronger SERT inhibition that cause these differentiated effects. **hypothesis**

She definitely needs to consult with her PDOC ASAP about this.
